Memory Lights the World: Create your name plaque and optional memorial page in a few simple steps.

Complete the form below and select the plan that works best for you.
Fill out the memory form with your loved one’s information: You will be emailed another form to enter the information of your loved one (name, date of birth and passing).
Only for package #2 and package #3: Submit the photo and tribute story. Our editor will review the submission and will contact you with any questions or to schedule your meeting times.

You will receive an email notification when your loved one’s memorial page is ready.

If you’ve selected package #3: our editor will contact you within 3 business days to schedule a meeting time.

The memorial page will be carefully reviewed by our editor, and we will reach out if there are any queries. Your loved one’s plaque will be posted within 5-7 business days.

An email with your tax deductible donation, along with the link to submit your loved one’s information will be sent to you shortly.
